It's not an illness at all. It's a surgical diversion of either the digestive tract or the urinary tract. The reason for it could be due to several different illnesses or even an injury. And everyone with an ostomy still has an ass hole. Just because they have an extra hole elsewhere doesn't mean they don't have an asshole.

Well, if it's not in 'the ass' then it's not an ass-hole, is it? True, in it's self, it's technically not a illness, but it's still classed as a 'hidden illness', or at least, in the UK it is. Also, since the original orifice is now sealed, my original point still stands- they don't have an 'ass-hole', they have a bit where it used to be.

The surgery isn't an asshole, but it doesn't remove the asshole, even when it bypasses it. The point is the person still has an asshole even if they aren't using it any more.
If an asshole has a brain but they're not using it, do they still have a brain? And what about entropy? Withering from lack of use?

There's a very long one by Etaski out there, won't link because it's not on here. It's more fantasy (LotR-like) than space based sci-fi. It's broken down into 4 or 5 different books and there are another dozen or so short stories providing additional character background as well.
You could try Charlie Foxtrot's recently concluded A New Past.
A disenchanted scientist is sent into a version of his past and given a chance to change his future. Can he use is knowledge to avert the dystopian future he has lived through or is he doomed to repeat the mistakes of his past?

If you're a sci-fi fan you might have already read Asimov's classic Foundation trilogy. Later, he wrote 4 other Foundation books, and there are other books in the same universe: Pebble in the Sky, The Currents of Space, and The Stars Like Dust. Since his death other authors, with the permission of the Asimov estate, have been continuing to write in the Foundation universe. Of course, since these works were published by mainstream publishers there are no graphic descriptions of sexual activities.

Every time I see Greenies referenced in a post, I have an almost uncontrollable desire to reread it. For about the umteenth time. Even with all it's warts, it is powerful.
Hamilton's "Deadnight" series. There is some sex, but nothing really graphic. Good for dead tree stuff and well written.
I'm just going to warn you that these stories are the dumbest things I've ever read. Just when you think it's reached the peak of insanity, it goes further. And yet, I couldn't stop reading them. The graphic sex tapers off throughout the stories but is extremely strong at the start.
Anyway, here is my favorite military sci-fi fantasy melodrama. The universe listing is slightly out of order but the descriptions and titles contain the proper reading order.

you might try Lonahora's book earths core. it has been going on got a long time. The only ptoblem I have with it the chapters are only about 10k in size and they are posted on Saturdays. I am starting to feel this is another story that the author has no idea how to end. But is has been enjoyable.
