Midori paused halfway through setting the mugs on the table.

It was such a small hesitation that Emiko might not have noticed it at all if the kitchen had not been so quiet.

Rain tapped at the windows. Hana sat at the table with her phone in one hand, half-listening as Emiko spoke about something she had seen at the market. Kaito leaned against the counter across the room, silent as usual.

Midori’s fingers tightened around the handle of the cup.

“Hm,” she said.

Hana looked up. “What?”

“Nothing.” Midori set the mug down carefully. “I think I caught my breath wrong.”

Emiko turned from the counter. “Are you all right?”

“I’m fine.” Midori gave her the quick, dismissive smile she used whenever Emiko worried too soon. “It just hurts a little when I breathe.”

“Where?”

Midori touched the left side of her chest, just beneath her collarbone.

“Here.”

Kaito straightened.

Not sharply. Not enough for anyone else to react.

But Emiko saw it.

“Did you fall today?” he asked.

Midori blinked at him. “No.”

“Get hit?”

“No.”

“Were you running?”

“Kaito, what is this?” Hana asked.

He did not look at her. His eyes remained on Midori.

“When did it start?”

“A few minutes ago, maybe.” She frowned. “Why?”

“Was it sudden?”

“Yes.”

Kaito crossed the room and stopped beside her chair. He did not touch her. He bent slightly, his head tilted, listening.

Emiko watched his expression change.

There was no alarm in it.

That was the first frightening thing.

His face became still—not blank, not numb, but focused with a precision that made the room seem to narrow around him.

“Kaito?” Midori said.

“Take a slow breath,” he told her.

Midori tried.

Halfway in, she stopped.

Her face tightened.

“It hurts.”

“I know.”

His voice was calm.

Too calm.

“Kaito,” Emiko said, “tell me what is happening.”

He looked up at her.

“There is air around her left lung.”

For a moment, Emiko did not understand.

“What?”

“It is not expanding correctly.” He turned back to Midori. “She needs to stay still.”

Hana put her phone down. “What do you mean, air around her lung?”

“A tension pneumothorax.”

The words fell into the kitchen with terrible finality.

Midori stared at him.

Then she laughed once, thinly. “That is not funny.”

“I’m not joking.”

The color drained from her face.

Emiko stepped closer. “How could you possibly know that?”

Kaito did not answer.

Midori took another breath. It came too quickly this time—shallow, panicked, useless.

“Kaito.”

“I’m here.”

“I can’t—”

“You can.” He lowered himself in front of her. “Small breaths. Do not force it.”

Her eyes widened.

“I can’t breathe.”

Hana stood so quickly her chair scraped against the floor.

“I’m calling an ambulance.”

“Yes,” Kaito said. “Tell them suspected tension pneumothorax. Sudden left-sided chest pain, worsening respiratory distress, possible pressure shift.”

Hana stared at him.

“Now,” he said.

She moved.

Emiko dropped beside Midori’s chair. Her daughter was sweating now, one hand gripping the table hard enough for her knuckles to whiten.

“Midori, look at me,” Emiko said. “Help is coming.”

But Midori was looking at Kaito.

“What is happening?” she whispered.

“You have air trapped where it should not be,” he said. “It is putting pressure on your lung.”

“Can you fix it?”

The question was quiet.

Kaito’s answer came without hesitation.

“Yes.”

Emiko turned on him. “No.”

He looked at her.

“No,” she repeated. “We wait for the ambulance.”

“She has minutes.”

“You cannot know that.”

“I do.”

The words were not raised. They were not emotional.

They were worse than either.

They were certain.

Midori’s shoulders curled forward. Her lips were beginning to pale.

Kaito glanced once toward the cabinet beneath the sink.

“Hana,” he said. “Get the emergency kit.”

Hana froze with the phone still at her ear.

“Now!”

She ran.

Emiko caught Kaito’s arm. “What are you doing?”

“I need a large-bore needle.”

“No.”

“Mom.”

“You are not putting anything into her chest.”

His gaze met hers.

He did not plead. He did not look frightened. His hands were perfectly still.

“If I do nothing, the pressure will continue to rise. Her circulation will fail. She will lose consciousness. Then she will die.”

Emiko could not speak.

Kaito held her eyes for one second longer.

“Hold her shoulders still.”

Hana returned with the kit, her hands shaking so badly that the metal case rattled.

Kaito took it, opened it on the floor, and selected what he needed. His movements were efficient and exact. He checked the needle, adjusted the improvised assembly, and then pressed the point lightly into the heel of his own palm.

Hana gasped.

Emiko stared at the bright bead of blood that appeared.

Kaito withdrew the needle and examined it.

“I needed to confirm it would not bend,” he said.

He set it down, made a minor correction, and picked it up again.

Midori’s head had begun to tilt toward her shoulder.

“Kaito,” Hana whispered. “She’s getting worse.”

“I know.”

He moved in front of Midori.

“Mom,” he said.

Emiko could barely draw breath herself. “What?”

“Hold her.”
