Serena

The neurologist’s office had the same bare winter tree in the courtyard window.

Serena noticed it the moment she sat down. Of course it was still there. Trees didn’t rearrange themselves between appointments. But she had looked at that tree the first time sitting in this chair while Dr. Reeves said the words glioblastoma and grade four and six months and she had filed it in her memory the way you file the details of a moment that changes everything. The tree was just a tree. It had nothing to do with any of it. She looked at it anyway.

Margaret sat to her left. Connie to her right.

Dr. Reeves was in his fifties, silver haired, with the careful unhurried manner of a man who had learned that rushing delivered bad news faster without making it any better. He had the MRI results on the screen beside his desk and he turned to them with the expression Serena had already catalogued from the first visit — composed, honest, not unkind.

His eyes moved briefly to her left hand. Then to Connie’s. He said nothing about the rings.

“The tumor has progressed as we expected,” he said. “Grade four glioblastomas follow a fairly consistent pattern in the early months. What we’re seeing is consistent with our initial prognosis.” He paused. “The location means we’ll begin seeing increased pressure symptoms over the coming weeks. The headaches will intensify. You may experience more frequent episodes of weakness on your left side. Possibly some visual disturbances — peripheral blurring, occasional double vision.” He looked at Serena directly. “Cognitive changes may begin to appear. Word retrieval. Occasional confusion. These will likely be subtle at first.”

Serena listened. She had gotten good at listening to this from a slight distance, the way you listen to a weather report for a place you’re going to visit and can’t avoid — noting the information without letting it fully land until later when you could be alone with it.

“How long before the cognitive changes become significant?” Connie asked.

Dr. Reeves looked at her. “It varies. In most cases with this type and location we see meaningful progression around the four to five month mark from diagnosis.” He paused. “You’re at approximately two months now.”

Two to three months then. Serena did the arithmetic without being asked.

“Is she in any pain currently?” Margaret said. Her voice was careful and level, the voice she used when she was managing something.

“The headaches,” Serena said before the doctor could answer. “Morning ones mostly. The ibuprofen handles them.”

“We can move to something stronger when that stops being sufficient,” Dr. Reeves said. “I’d like to adjust your anti-seizure medication slightly. The current dosage may need support as the pressure increases.” He looked at Serena. “How have the seizures been?”

“One small one last week,” Serena said. “My hand. It lasted maybe twenty seconds.”

Margaret made a small sound beside her.
