I clicked on it and a box appeared that said "add note to story" and "save note". Not something I will use...

I've been known to browse through my history and find stories that I haven't read for ten years, just because I don't remember them. Having a note that said - read, worth re-reading later or one that said bovine excrement, don't bother - might be useful.

Just because I voted a story an 8 may mean it was good for technical reasons. Some stories are quite well written, and I just didn't like it for a personal reason. I've got close to fourteen years worth of stories I've read in my history!
It's a new feature. Those are notes for yourself. They also show your own tags if the story is bookmarked and tagged.
You can turn them off in the library's preferences page, line #19.
