@Dominions SonActually, the exception are the covers, as their whole point is to draw in those who've never read the author before, as those who have would purchase the book anyway. So, at least within the publishing community, as long as it reflects the basis of the story, whether it reflects a particular scene or not is of little concern.
Which for me, works well, as I mostly rely on fairly genetic stock-art images, which reflects the overall theme of the story. But for the images IN the story, most readers have higher expectations, so it it doesn't precisely fit that particular scene, they'll rake you over the coals for it. (I've always designed my own covers, but have only occasionally included artwork in any, and have only included one piece of my own artwork, as it fit the descriptions of the basis of artistic styles, which few outside of the field could easily grasp.)
