I didn't happen to see that title, but seeing a few titles that are not showing the proper cover picture on my kindle app in tile mode, but I do see a wide aspect ratio cover picture when I open the title. The tile preview looks just like a basic DOC file. Similar to what was shown before the rollout of picture based covers (counting the generated text only covers as pictures as they appear in the tiles that way.
Titles downloaded by Barbar are the most recent examples, but not unique to this author.

The file that is sent to the kindle is a standard EPUB file as Amazon had dropped support for .mobi that we used to send.
My CMS's EPUB creator creates files that are very standard compliant and error free generally.
How Amazon's document mailing system converts them from EPUB to whatever internal format they're now using for the Kindle is out of my control, so whatever shows up on your kindle is up to Amazon.
That being said, I just tested one of barbar's stories using my own Kindle (Paper white 2) and the cover in the library showed up as expected. And when I opened the book it was there too.
So, for me, on a kindle paper white, it's working as expected, nothing out of the ordinary.

It's the kindle app inside of iOS and iPadOS. Issues are consistent regardless of os and kindle updates. Issues have existed, but only for some titles, for years. Barbar was just one example of images that won't show miniature images in gallery view but does show the cover art when opened.

If all you use is an iPhone and an iPad, then why not simply download the EPUB and use the Books app to read?
Because the email to kindle is able to send to my cloud kindle account and I can use the app on any of my iPad or iPhones or my kindle app on a laptop and not have to manually sync the files across my devices

Apple Books syncs to the cloud. I load once on my iPad and it's available almost instantly on my iPhone and both my Macs.
