DuckDuckGo keeps changing my safe search from "off" back to "moderate." Not always, but often enough. Anyone know how to make my change permanent (at least until I make the change)?

For more technical users, if you want to force safe search on (or off) you could use our URL parameters and automatically append &kp=1 (strict), &kp=-1 (moderate - don't show explicit results), or &kp=-2 (off) to all duckduckgo.com pages

Choose "Other settings" from the menu. After you have set all your preferences you click on "Show Bookmarklet and Settings Data" on the right side of the page.
It will show "Bookmarklet URL" which looks like "https://duckduckgo.com/?kp=-2&kz=-1&kc=-1&k1=-1&kaj=m&kam=osm&kak=-1&kax=-1&kaq=-1&kap=-1&kao=-1&kau=-1&kae=d&kn=1".
Copy that complete line and set is as the URL you use to open DDG (properties of your DDG browser bookmark).
One of the codes in that line will be like what Zellus mentioned in his post.

Actually, it didn't work. It turned the search back to "moderate" even though the URL had the ?kp= -2

There's an option to save settings in the cloud. If you choose that it asks for a pass phrase. Don't really understand that. It's not like I'm sharing my settings on multiple devices. It did say something about the settings being set in cookies so if your cookie is deleted and it's saved in the cloud then it's not lost. But it seems to get lost without deleting cookies.

I use the URL with the codes in my desktop browser bookmarks like I described (copied from the DDG settings page) and I never have a problem with it. Is it only the safe search option that doesn't stick? Try configuring with family filter off and dark theme, set that URL in your browser bookmark. With the dark theme it's easy to see if you have retained your chosen settings.
Make sure you click the "Show Bookmarklet and Settings Data" on the settings page so you can copy the URL under "Bookmarklet URL".

That's the only one I changed from the defaults.

I did bookmark the URL from the "Show Bookmarklet and Settings Data." Now I made additional changes and bookmarked the modified URL. (I didn't see "family filter.")

The safe search option, often (mistakenly) called "family filter". Sorry for the confusion.

When and if I need the safe search off, it's a couple of clicks, and stays that way for the session. Otherwise I don't worry about it.

I find this often with many sites (most often on the Adobe Stock site, where every new search immediately defaults back to "safe search".

With AS, it's really ridiculous, since they hardly have ANY racy material at all, but I don't like to limit my options, when it's already so difficult finding any viable images that are 'spot on' perfect for a given story. But the policy seems similar to most sites, where the default is 'always on' safe-mode.
