if it's for an existing story, simply submit the cover image and put in the moderators notes field that this is a cover. We'll insert it at the start of the text.
If it's for a new story, submit the cover with the text and put in something in the text where the cover should go that mentions the attached image. We'll handle it.
When I deploy the changed cover support, the wizard will have an entry for cover image and the story management page will have an option to upload a cover and it will be handled by the system automatically. So displayed on the site and used as cover for the EPUB file.
