I'll look it up, but someone had a MC die, and the 'afterlife' for him was living on a planet run by gods from various pantheons, where they had followers travel into a D&D/Diable style dungeon.
Another story, Godless&Faithless, IIRC, had two friends transported to a fantasy RPG-inspired world.
Then there's a few decent D&D-type stories.
