Some of mine run in a world were the Cold War didn't end in the end of the 80s/beginning of the 90s. Other than that, that world kind of runs in the background and isn't that different.

You could apply this thinking to The Swarm Cycle universe. If you're unfamiliar with it, the series involves aliens giving humans tech to fight a war, but having a lot of restrictions to who can leave the planet. The off-world human culture follows a model of sexual slavery where people, mostly men, who are good at combat are the masters and everyone else has no legal rights at all.
The more you know about psychology and morality, the less like the real world any of this is. For example, in a testing system that values responsibility and intelligence over aggression, there are 5 times more male masters than female because... it's never really explained. But somehow men in this universe seem to be automatically smarter and more responsible if they are also aggressive and interested in extreme BDSM.

Edit: * - I'm re-reading Kennedy and I had forgotton how much of the story is about ff (and Ff) sex, it was written around 2007 which is way before the under-14 restriction was introduced on this site. The story is also "Buffy the Vampire Slayer" fanfic and I have never seen the original.
Another Edit: I withdraw the suggestion you read "Kennedy" - it is not really "alt world". While parts are quite good, other parts are borderline unreadable.
