It's a term that's been in use since the 1600s at least (although I've seen others reference it dating back to the 1400s), and is based off a no longer used definition that meant "relating to the state or it's citizens" which was itself from the French Civile (thanks to the Normans) which came from the Latin Civis and Civillis, which mean citizen.
So in the 1600s context when the word first started being used, Civil War meant a war between citizens.
It's also why we call it "Civil Rights", not because they're polite rights, but meaning rights of citizens.
