Most of what I remember is in the subject. The 'terminal patient girl in hospital' setting is just a prologue, so to speak. She persuades her (male) best friend to go into this new world with her ... it is not clear from the beginning of the story whether this is a dream or an alternate reality or what. I found the story again some time ago but I can't seem to find it again as of this month.

I remember one very close to this, but the girl (who was very close to death) was actually recruited by the guy (and his sister/girlfriend - it was complicated). Guy was the MC, ends up being most powerful mage on the planet, his 'sister' was a healer/mage. The country they were in was about to be at war, had normally four major royal wizards to protect the country, each who recruited their own students. MC was recruited from earth, he led the recruitment of dozens more to prepare for war. Any chance that's the story, or was it another?
I remember the story, but not its title or author. I believe it was never finished.
I suspect it might have been 'Best Friends' by Rhyshadow, but the story and author both seem to have disappeared :-(

I think that was also a thing in "Apprentice Tremaine" by Rotedrachen, when they recruited people (including a girl with terminal leukaemia) with magical potential. In the middle of the story, I believe.
Yeah, that's the story I was thinking of. I hadn't remembered the title, and didn't know if it was worth looking up...

Thank you everyone. The MC here was the girl and I don't recall the guy having a sister/girlfriend - or rather, he had a girlfriend and wondered what was going to happen to her while he was 'away' but she encouraged him to participate.

The funny thing is, I kept running into it for a while... I'll look to see if it got stuck in my library and will update.
