In general, I would assume that "safe sex" implies protection from transmission of sexually transmitted diseases as well as pregnancy.
In fact, I would say that the disease issue is more important for "safe sex" than birth control.
Now this doesn't necessarily have to involve condoms, it could for instance involve partners abstaining until they have both been tested and have a clean bill of health.

If you are only considering pregnancy then any reliable form of birth control applies such as the ones you mention (condoms, diaphragms, the pill, having had a vasectomy, the implant...)
If you are also considering STDs, there are other measures that might apply, including the use of condoms if there is any doubt that their partner is safe.

Essentially it means to keep your willy in your pants until you're certain you won't be dippin' it into a diseased sump. And vice versa. If yer willy is a festerin' stump, keep it in a baggie and don't be dippin' it into anything short of a bucket of molten sulfur to sterilize it. Ergo: safe sex.
P.S. That might also apply to diseased tongues, festerin' fingers, and weepin' anal orifices. Can't be too careful these days.

There's a show on Cable called Sex Sent Me to the ER. The have all kinds of amusing stories.
One included a couple who had sex on a neon sign (A large cutout shape of a cowboy that sat at ground level but was 30+ feet high in total. They ended up in the ER with both of them having electrical burns to their genitalia.
They thought they had been electrocuted by the sign. However, when they were brought into the ER, a number a medical professionals got shocked when they touched the male.
It turned out that the male (late 20s to early 30s) had a recently implanted pacemaker which was miss-calibrated. It kept going off when it shouldn't and at a higher voltage/current than was needed.
I was just beginning to wonder if it also covered people having sex in a walk-in safe as well.
Walk-in safes are only covered only if there is a way for those using that location for a sexual encounter to either alert someone outside that there are people trapped in there, or a way for someone who is authorized to be inside the safe with the door locked to exit it. ;)

It you splunge all over the insides, cracking it open is the ONLY way to get your money out! Apparently, this particular behavior has been going on for a very LONG time (i.e. it's didn't originate with the 1960's 'free love' movement).
I've always assumed it means "a story that deals with issues of protecting those you lose from sexual health issues". Thus, as long as that's a theme in your story, whether trying to control accidental pregnancy or protecting yourself ("You'll never get me, Copper, I'm armed with a fiver-inch penis and I kinda know how to use it!"), you should include it.

I meant, the two thoughts generally tend to accompany each other. Readers who are upset by 'casual sex' frequently take comfort when they see 'safe sex' practices in stories. They aren't particularly upset at orgies, harems or sleeping around, but then want to see some level of commitment (i.e. at least caring about the other person, even if it goes nowhere).
Safe is the opposite of sound, as in safe and sound, so safe sex means noiseless sex so the parents can't hear you ;)
